Please follow the instructions below to complete the 1st stage of the online DBS process.

It is very straight forward however please contact me on volunteersecretary@carefarnham.org.uk or 01252 715110 should you experience any problems completing the process.

• Using browser connect to uCheck website at www.dbsassist.co.uk/ucheck/
• Select ‘Start Application’
• Login: Organisation ref enter ‘CARE42’; Password enter ‘carefourtytwo’
• Enter your data: Name, DOB, Nationality, NI number, Contact details
• On completion select ‘Next’
• Enter your data: Address or addresses must cover a period of a least the last five years
• On completion select ‘Next’
• Enter your data: any other names used with dates since birth to current, convictions select ‘No’
• On completion select ‘Next’
• Sign and submit
• Select home then disconnect browser from the website

On completion of the 1st stage I will be notified by the system and can then complete my part once I have received your documents.
